Care pathways

One of the most important tools for acute pathway comprehension and redesign is understanding the pathway, processes along the pathway, duplications, blockages, loops and repetative cycles. This section contains examples of pathway comprehension design and examples to assist different options. It also links closley with audit, variance, standards and monitoring.

	Title: Care pathway guidance Published: April 2004 Summary: The York Acute Inpatient Pathway Guidance for Nurses. Ward managers and other staff have helped construct an inpatient pathway for Bootham Park Hospital. This brings together the range of interventions that should be provided and groups them into the various stages of a person’s hospital admission. If followed, it ensures that service user’s receive the optimum care whilst in hospital and increases the likelihood of a speedy and safe return home.
